Attention: Parents of 6th Graders!

You should have received a letter and consent form with your child's report card concerning a NC immunization requirement- Meningococcal Conjugate Vaccine.

In order to provide easy access to this required immunization, Bladen County Health Department will provide a clinic at Clarkton School of Discovery in March.  If you would like to for your child to receive this vaccine at the school level clinic, you must complete the consent form and return it to your child's teacher.
